Patient Insurance plans vary significantly and therefore we are unable to have knowledge of each plan’s specifics. It is very important for you to be aware of the requirements of your own health insurance policy regarding eligibility, referrals and/or authorizations that may be needed prior to your procedure.

Patients will be responsible for deductible, copayment, co-insurance, and a facilities fee as specified by the patient’s plan.
